Patient Forms

Upon entering our office, our receptionist will welcome you as a member of our office. We will request that you complete our patient forms that will be emailed to you prior to your appointment to give you and the doctor enough time for the exam and treatment. This paperwork is also available in person. We ask that you show up 15 minutes prior to your scheduled appointment time if this is your preferred method to complete the forms. This paperwork provides us with your health history and information on your condition.

Detailed Initial Evaluation

Next, you will have a 45 minute to 1 hour consultation and exam with Dr. Demers or Dr. Bruner.  During this, we will discuss your current condition and health history to rule out underlying conditions and need for further workup outside of our office.  We will be also discuss activities in Work, Home and Leisure life to try to find any damaging or contributory activities. This initial visit is designed for the doctors to learn more about you, your condition, and expectations to determine how chiropractic care can meet your goals.

Report of Findings

Once the information is collected and examinations are performed, Dr. Demers or Dr. Bruner will give you a detailed report of all findings and answer any questions including:

  1. How can you help me?
  2. How often do I need to come in?
  3. What will my treatment cost?

After reviewing your health history, goals, and examining your spine and X-rays, the doctors will discuss recommendations and notify you if your condition requires care with other providers. At Harrisburg Joint & Muscle, the doctors and staff will provide the best treatment and wellness program for your needs.
